In his letter to the National Board of Medicine, Dr Goldkuhl wrote:

The patient, who on her father’s side has a hereditary predisposition to mental illness, is quiet and reserved by nature. Although she never had an especially robust constitution, she previously always enjoyed good health and presented as happy, industrious and engaged. Entering her 27th year, following two childbirths in fairly short order and the shock of a sudden death in the family, she began to sleep poorly, became taciturn, withdrawn, disengaged, forgetful, was unable to perform her domestic duties as before, became morose and self-accusatory, lost her appetite and lost weight. After this had been going on for about a year, she committed an act entirely at odds with her natural interests and previous disposition, which lacked any motive and was decidedly impulsive in every respect. Immediately thereafter, her emotional side became completely dead. She has since become even more silent and reserved than before and is displaying partial amnesia. She gradually brightens up enough that she can feel remorse and be reasoned with, but still exhibits, at the time of writing, catatonic and demented traits. The diagnosis should not be anything other than catatonic schizophrenia with depressive episodes.
